This is a personalised service that provides continuous care to the highest level and access to the latest in education and technological developments within the field of diabetes and obesity management.

London Diabetes offers a different approach to the management and care of diabetes. The main guiding principles are:

• To agree a treatment plan for a patient that is achievable, appropriate for the patient needs and payment options that is personal to the patient and not defined by a government or central guidelines
• To ensure that a patient has access to education appropriate to their needs, so that they can participate in their management and understand how they can contribute to an improvement in their health
• To be innovative in diabetes care, offering but not necessarily instituting the latest and best available safe therapies
• To offer support in a patient’s journey with diabetes and be their champion if and when they are not helped

• A once a year consultation with a consultant diabetologist (foot check included) with goal setting
• One teaching course per year
• Regular updates and internet teaching modules and news
